VISION INSURANCE
The following is a list of the Medical Insurance companies we are providers for.
So that you can be informed of the coverage your Medical insurance provides for you, please call your insurance company at the phone number provided on the back of your insurance card to ask if your plan covers “Routine Eye Exams” and also if you have any benefits for “Materials”.
We participate in the following Vision Insurance plans,
We are out-of-network providers for Superior Vision, Davis Vision, NVA, and Spectera.
Call for our current promotion. We will provide an itemized receipt with the out-of-network reimbursement form,
so that you can submit them to your insurance to get reimbursed.
If you make an appointment by phone, please have your insurance information available.
